Multimorbidity and Polypharmacy in Psychosis: Identifying Potentially Significant Drug Interactions in an Old Age Inpatient Psychiatry Setting

Aims To determine potentially significant drug interactions in an old age setting among inpatients with psychosis and medical comorbidities over the past 12 months Methods Study setting: inpatient functional ward Sample size: 11 Inclusion criteria: • Inpatients on psychotropic medications diagnosis ranging from F20 to F29 (ICD-10) Comorbid chronic condition(s) requiring long term treatment Exclusion Patients other/co-existing mental health excluded Antibiotics , OTC medications, herbal treatments, emergency treatments other short-term administration analysis tools: Lexicomp® interaction checker ◦ Category X: Avoid combination D: Consider therapy modification Further categories (In order of decreasing risk ratings: C, B, A) Results Out total 67 admitted between January 2022 December 2022, 17 patients had F29. 4 were due no 2 lack data. Among included analysis, most common were, order, hypertension, hypercholesterolemia pain. A 114 noted physical as follows: No interactions: (category A, C): 103 D (consider modification): 10 7 these associated administering Buprenorphine (Flupentixol, Gabapentin, Nortriptyline, Olanzapine, Amisulpride, Aripiprazole Pregabalin) leading additive CNS depressant effects. Interaction codeine Valproate Flupentixol carried depression. Carbamazepine interacted Risperidone via CYP enzyme induction. X (avoid combination): 1 The Glycopyrronium was severe anticholinergic effects Conclusion This study is a preliminary investigation towards understanding population ailments. Awareness regarding potential can help avoid combinations possible detrimental limitations this study, terms sample size quality available data, be overcome by conducting planned larger population.
